Instructions for Referers

Electronic referral form

1. Click on the electronic referral form to download it to your computer.


2. Keep a copy of the electronic referral form on your Desktop so you can easily copy and paste information from you notes into the various sections of the form. There is a drop down menu for the patient's conditions and you can manually type information in by first clicking on each of the sections.


3. Save the completed form so you can keep a copy for your records. Then email it to rhermann@footandankle.com.au as your referral.

Standard referral form

• Print the standard referral from and hand write information into each section. This can then be scanned and emailed to rhermann@footandankle.com.au


• You can fax either form to 07 3054 3131 (be sure to include the 07 as it will come directly to Dr Hermann's email via your fax).
